2011 Certificate of Coverage Prior Authorization

  • Prior authorization requirements may apply to either Network or Non-Network services, or both, depending on the specific benefit category. Prior authorization is required 5 business days before a scheduled admission. Notification is required as soon as reasonably possible for non-scheduled admissions.
  • Benefit penalty for no authorization: Depending on the type of service, coverage may be reduced to 50% of eligible expenses or no benefits will be paid if the member fails to meet their prior authorization requirements. Note: Penalty only applies when there is member responsibility for prior authorization.
  • In addition to Prior Authorization requirements, the member is required to contact us 24 hours before Non-Network scheduled admissions. However, the member will not be penalized if this notification requirement is not met.
